Dairy Queen - accuracy of order and fan survey access

Hello DQ folks-
Two items:
1) Order accuracy - I carefully and specifically ordered a large Blizzard (among other items). The item I got was not even close to what I ordered. I ordered mint, chocolate chunk. I got mint oreo cookie. Into the trash.
2) I tried to access the DQ fan survey to comment on this. Twice the system denied me access claiming that the information I entered did not allow me to proceed. Really? Survey code [protected]-724321. Then the site would not allow me to access any option - including contacting DQ.
